Dr. Levinger Eyes Regulations – Laser Vision Correction Surgery

Suitability test

A suitability test is a comprehensive vision examination specifically intended for laser vision correction surgeries (treatment code 71).

The suitability test is performed at no cost. “Dr. Levinger Eyes” reserves the right to price the test at any time.

For a first suitability test, with low prescriptions under 18 or +5, the test will be done at no cost and without commitment.

Consultation test – A patient with eye-related medical history, such as: eye conditions, eye surgeries etc. cannot undergo a suitability test under the scope of “laser vision correction”, but only as a paid “consultation” test (treatment code 26).

Complex suitability test – patients who wish to undergo a suitability test and have the following[1] eye[1] characteristics: prescriptions of 18 and above or +5 and above, will be suitable for a “complex” suitability test.

The cost of this test (treatment code 991234) is ILS 500.

The cost of the test is offset from the surgery cost – that is to say, a patient whom had scheduled and undergone surgery will have the cost of the test offset from the payment they made for the surgery.

A complex suitability re-test carries a cost of ILS 500, offset from the cost of the surgery.

A “complex re-test” is a test that is scheduled if the physician requests a repeat of the test, or at the patient’s initiative (treatment code 991235).

A “suitability re-test” is an additional test conducted after the first suitability test, whether the physician requests a repeat of the test or at the patient’s initiative. This test carries a cost of ILS 200, offset from the surgery cost.

The cost of the test will not be reimbursed if surgery was not scheduled.
If surgery was scheduled, the cost of the test will be offset from the surgery cost.

The type of test will be determined in accordance with the results of medical questioning conducted by telephone or by WhatsApp, that the patients will undergo at the information center.

A cost shall be charged for the test in cases where potential for surgeries other than laser vision correction surgery is identified.

A second suitability test for a patient found suitable for laser vision correction surgery carries a cost of ILS 200, offset from the surgery cost.

A third suitability test carries the cost of ILS 500, without offsetting from the surgery cost.

A complex first suitability test for high prescriptions above -8/+5, and for patients ages 50 and up, carries a cost of ILS 500, offset.

A patient deemed suitable for surgery who wishes to undergo a test again – the cost is ILS 1200. Private consultation test – ILS 1200, valid for 3 months.

A third complex suitability test carries the cost of ILS 500, without offsetting. Post-operative check-ups

Over the first three months, the patient will have to arrive at check-ups per the operating physician’s instructions.

The cost of each check-up after one year following the surgery is ILS 500.

It is the patient’s responsibility to schedule annual eye examinations at the HMO / with a community physician to maintain the health of their eyes.

* The patient must provide full and accurate details during the medical questioning by telephone / WhatsApp at the information center.

Should any information that was not be provided during the call be discovered in the findings of the test, the clinic may charge for full medical consultation at the time of the clinic visit, even if the patient was not informed of this by telephone.
