What is oculoplastics?
Oculoplastics is a specialized field in ophthalmology, treating the area of the eyelids, eye socket, and tear ducts, also known as plastic surgery of the eye.

Droopy eyelid correction (ptosis)
Eyelid lifting surgery is a procedure of great medical importance, as a droopy eyelid causes restriction of the visual field, strain on the forehead muscles leading to headaches, and may cause fatigue in the eye area. The reasons for upper eyelid drooping are: weakness of the muscle that lifts the eyelid, excess skin, or drooping of the brow.
Upper eyelid surgery
Excess skin between the eyelid margin and the eyebrow results from loss of skin elasticity, loss of subcutaneous tissue, and drooping of tissues above the eyebrow. This excess skin is sometimes accompanied by protrusion of fat from the eye socket, which stretches the skin outward and creates bulging bags above the eye. Drooping orbital fat can be removed from the upper eyelid during upper eyelid surgery (blepharoplasty).
Lower eyelid surgery
The lower eyelid is a relatively complex organ: the proximity of the lower eyelid to the center of the face and the cheek causes these parts to depend on it, and consequently, the forces that act on the face affect the shape and general appearance of the face. In addition, the tear ducts pass through the lower eyelid, so any surgical procedure requires extra caution in the surgical approach and a deep understanding of the structure and function of the mid-face and lower eyelids. Typical problems of the lower eyelid include excess skin, protrusion of orbital fat, and the appearance of “bags” under the eyes, or loss and depletion of subcutaneous tissue, and the appearance of hollows under the eyes. In lower eyelid surgery, an incision can be made from the inner part of the eyelid so that no scar is visible at all. And in situations where excess skin is removed, an incision can be made below the lash line, so that the scar is minimal.
Xanthelasma
The appearance of yellow masses in the upper and lower eyelid or at the sides of the eye. This phenomenon may appear in combination with excess cholesterol and fats in the blood. Xanthelasma can be removed by local surgery. However, as a preventive treatment, it is recommended to maintain low levels of fats in the blood.
Removal of benign lesions in the eyelid area
The accepted approach for benign lesions is to document the lesion and regular follow-up by a specialist doctor. If the lesion is cosmetically bothersome or if there is any change in the lesion, whether objective or subjective, it is recommended to remove it and conduct a pathological examination.
Treatment for tearing
Tearing in adults is a troublesome phenomenon, more common in women. Usually, the reason for tearing is blockage of the tear duct due to trauma, sinus disease, nasal polyps, inflammation, infections, and rarely – tumors. In infants, in 90% of cases, the phenomenon resolves by the age of one.
- Widening of the tear duct opening (punctoplasty)
- Opening tear ducts in adults and children – Mechanical opening of the tear duct using a delicate tube and/or widening the tear duct with a balloon.
- Tear duct bypass surgery (DCR) – Surgery to create a bypass that will drain tears from the tear sac to the nasal mucosa through a hole made in the nasal bone that replaces the blocked duct. The surgery is performed under general anesthesia.
Surgeries to correct eyelid position
- Entropion – eyelid turned inward. In this condition, the margins of the upper or lower eyelid and the eyelashes have turned inward and are pointing into the eye as a result of aging or trauma. The reversed growth direction of the eyelashes causes corneal rubbing and pain.
- Ectropion – eyelid turned outward – In this condition, the edge of the eyelid is facing outward, and as a result, the conjunctiva is exposed to air and dries out, and the edge of the eyelid is red and prominent.
